Hearing Dog Application Process

Please complete our online Hearing Dog Application request form.

2.
Within one week, you will be mailed our complete Hearing Dog Application and additional forms to be completed and returned to us.

3.
When we receive your additional forms, we will notify you to schedule a home visit with an instructor and dog-in-training. This process may take up to 1-2 months depending on your location and staff availability. We strive to create a safe,comfortable space in which to ask questions and understand how a Hearing Dog can best help you.

4.
If our program is not going to be the best fit for you, we will make other recommendations where we can.

5.
Approved applicants will be placed on a wait list and we will check-in by email every 3 months until you are notified of a dog match. The waiting time to be matched with a dog can vary but in general may take up to 1 year. This is subject to change.
